Chemical Name: Trimiparamine Maleate Brand Name: Surmontil Manufacturer: Wyeth-Ayerst Description: Mechanism of action is unknown, but a tricyclic antidepressant that increases the amount of norepinephrine, serotonin, or both in the central nervous system by blocking their reuptake by the presynaptic neurons. Indications and Use: For the relief of symptoms of depression. Contraindications, Warnings, and Precautions: Contraindicated during acute recovery phase of myocardial infarction and in patients with hypersensivity to drug or receiving MAO inhibitor therapy within 14 days. May interact with barbiturates causing decreased TCA blood levels, monitor for decreased antidepressant effect. Use with cimetidine, fluoxetine, sertraline may increase serum trimipramine levels. Clonidine, epinephrine, and norepinephrine may have an increased hypertensive effects. Avoid concomitant use with CNS depressants. Do not use with alcohol. Avoid unprotected or prolonged sun exposure. May increase risk of photosensitivity. Adverse Reactions: Possible side effects my include drowsiness, dizziness, orthostatic hypotension, tachycardia, blurred vision, dry mouth, constipation, urine retention, and diaphoresis. Possible life threatening side effects may include arrhythmias, heart block, myocardial infarction, stroke, and hypersensitivity reaction. After abrupt withdrawal of long term therapy: nausea, headache, malaise (this does not indicate addiction). Dosing Range: In adults, the initial dose may be 75 to 100 mg per day. This dose may be increased to 200 -300 mg daily.
