Chemical Name: Doxepin hydrochloride

Brand Name: Sinequan (available in generic)

Manufacturer: Roerig Division of Pfizer Inc.

Description: Doxepin is a tricyclic antidepressant. Its exact mode of action is unknown. It is believed that Doxepin acts to prevent the reuptake of norepinephrine at nerve endings.

Indications and Uses: Doxepin is indicated for the treatment of depressions and or anxiety.

Contraindications, Warnings, and Precautions: Doxepin is contraindicated in patients with a known hypersensitivity, patients with glaucoma, and patients with a tendency to have urinary retention. Safety in pregnancy and nursing mothers has not been established. Doxepin should not be used with MAOIs (Monoamine Oxidase Inhibitors), Cimetidine (Tagamet) or with alcohol. Since Doxepin is one of the more sedating tricyclic antidepressants, it should be given at bedtime, and patients should be cautioned against driving or operating heavy machinery.

Adverse Reactions: Possible common side effects are drowsiness, dry mouth, nausea, and constipation. It may also cause blurred vision, urinary retention, dizziness, tremors, weight gain, and lowered libido. Photosensitivity is common, hence a sunscreen should be used. Stopping Doxepin abruptly after prolonged treatment may cause withdrawal (rebound) symptoms such as  nausea, headache, and malaise.

Dosing Range: For most patients, a daily starting dose of 75 mg is recommended. It may be increased slowly up to 150 mg per day or in more severe patients up to 300 mg per day. Blood levels are recommended.
